Office forms below are a great way to save time before your arrival to our office. Please download & save the forms first and then fill them out- YOU MUST SAVE TO YOUR COMPUTER BEFORE FILLING OUT THE FORMS. Then, you may email them to us at frontdesk@hipkidspediatrics.com prior to your first visit. There may be additional forms or items you need to complete at your child's appointment.  If you are a new patient, we require your insurance information, immunization records and a release of records to request any old records from your previous primary care office prior to scheduling with us. PLEASE PROVIDE AN INSURANCE CARD & COPY OF IMMUNIZATIONS WITH YOUR PAPERWORK to frontdesk@hipkidspediatrics.com and then call or email us to schedule an appointment. Thanks for your cooperation.
